Your Rights Regarding Personal Health Information

You have the following rights regarding your Personal Health Information:
- Rights to Inspect and Copy. You have the right to inspect and copy your personal health information. We may not be able to provide health information that includes psychotherapy note, as part of a legal case, or is otherwise excluded from disclosure by law. We may charge a fee for the cost associated with your request.
- Right to Amend. You may ask us to amend your personal health information if you feel it is incomplete or incorrect. We may deny your request if it is not in writing or does not include a reason to support your request. If we deny your request you may send a written notice of disagreement with our denial.
- Right to an Accounting of Disclosures. You have the right to request a list of you disclosures since April 14, 2003. The list may not include disclosures authorized by you; disclosures for treatment, payment and health care operations; or other disclosures authorized by law.
- Right to request Confidential Communications. You have the right to make a reasonable request of how and where we communicate Personal Health Information regarding treatment, payment or health care operations.
- Right to a Paper Copy. You are entitled to receive this notice in written form. You may request a paper copy of this Notice at anytime.
Other use or disclosure of your protected health information require your written authorization. You give us your authorization, you may cancel by writing to our Privacy Officer at the address listed. If you cannot give your authorization due to an emergency, Ashmat Nurse Registry may release your health information if its is in your best interest. |
